Though META Keywords and Descriptions aren't really highly regarded by Google, they are still part of an overall sound SEO strategy, and as others have mentioned are still relevant to search engines like Yahoo and Bing. It doesn't really take a whole lot of time to add these to your site, so why not just do it.

Meta tag is an important part of seo. I would recommend no more than 5 keyword phrases in the meta keywords tag, though you would also be fine to leave it blank. The search engines do not look at this tag anymore.

I don't think there's a strict limit regarding the keyword density for the page title and meta description. As for the title tag, you should create it for both users and search engines. The page title is taken into account by Google when ranking websites - so make it keyword-optimized, yet natural. Moreover, the page title is displayed as the first line in search results, so make it appealing for the user. As for the meta description, write it with just users in mind - SEs do not consider meta description when evaluating websites, but users read it in search results under the title tag - make sure it is appealing!!
