And now, with Panda's influence:
1. Borrowed, paraphrased or sloppily written content.
2. Excessive ads on a page
3. Ads that push content down

Even before the panda update those were already mistakes. Plus, only a SPAMMER and not a SEO will copy or paraphrase content from other websites.

Not only thing which you mentioned earlier but also duplicate content ruins the whole effort and get it banned.
